The Claim Process
The following Midwest Security authorization and request forms are provided for use by our health plan members. Members may use these forms to authorize release of their private health information or to request other actions concerning this information. The forms are in Portable Document Format (PDF). After opening the PDF, you can print the form by clicking the printer icon displayed on the Acrobat Reader menu bar directly above the displayed form.
